If you are interested in participating in the 2009 4-H Summer Camp, please complete the application form and appropriate medical form.  To receive a T-shirt, application is due  by June 1, 2009.  If you are not interested in receiving a T-shirt, completed forms are due by June 8, 2009.

Cost for youth not enrolled in 4-H is $12.00 for 4-H insurance in addition to the camp fee.  Adults not enrolled in 4-H pay a $6.00 enrollment fee in addition to the adult camp fee.
